We also require a completed credit card authorization form to be submitted to us at least one week prior to your first visit. This is to secure your appointment time, and to ensure that we receive adequate notice (48 hours) if you need to cancel your first appointment. It is only in the event that you miss your appointment, or fail to provide adequate notice that your credit card will be charged. The form can be mailed, e-mailed, verbally over the phone, or dropped off in person during our hours of operation.
